Filing a Complaint

Medical services on ElliotMeds are provided by Stotland Medical LLC, which is responsible for your medical care and is the custodian of your medical records. If your concern is about the care itself, our clinicians page lists how to contact Stotland Medical LLC directly.

For anything else, including a request for a copy of your records, write to [email protected].

We will not retaliate against you for filing a complaint with a licensing board.

A complaint about a clinician's professional conduct is handled by the licensing board in the state where you were located at the time of your visit. It is not handled by ElliotMeds. Which board applies depends on the clinician's credential.

Mitchell Brandon Stotland, MD is a physician. Complaints about his care go to the state medical board — in some states called the board of medicine, the board of medical examiners, or the medical licensing board.

Angie Chang, APRN is an advanced practice registered nurse. Complaints about her care go to the state board of nursing.

Both clinicians are listed on our clinicians page with their license numbers, so you can identify the correct board and reference the correct license when you file.

Every state operates its own board with its own complaint process. Two national organizations publish current contact information for all of them.

For state medical boards, the Federation of State Medical Boards maintains a directory of every state medical and osteopathic board. Each listing includes a direct link to that board's complaint form.

For state boards of nursing, the National Council of State Boards of Nursing maintains a directory of every state board of nursing. Select your state to reach that board's contact page.

Boards generally ask for your name and contact information, the clinician's name and license number, the dates of care, and a description of your concern.

You have the right to a copy of your own medical records. Our Notice of Privacy Practices explains how to request them.

A side effect or adverse reaction is reported through our adverse event page, and to the FDA. If you are having a medical emergency, call 911.
