Limitations and Risks

I understand that telemedicine has certain limitations:

Patient Responsibilities

As a patient, I agree to:

Provider-Patient Relationship

  • My provider has the right to deny or discontinue treatment at any time based on medical judgment
  • I have the right to refuse treatment or seek a second opinion

Privacy and Security

I understand that:

  • My health information is protected under HIPAA regulations
  • Electronic transmission occurs over secure, encrypted channels
  • Despite security measures, there are risks inherent in electronic communication
  • I am responsible for maintaining the confidentiality of my account credentials

Prescription Policies

I understand that:

  • Prescriptions are issued only when medically appropriate
  • Not all patients will be approved for treatment
  • Controlled substances may not be prescribed via telemedicine in certain jurisdictions
  • I am responsible for the proper storage and use of prescribed medications
  • Unused medications should be disposed of properly

Emergency Situations

I understand that:

Right to Refuse or Withdraw

  • Request consultation information be provided to my primary care physician

Consent Acknowledgment

By completing the intake process and using ElliotMeds services, I acknowledge that:

  • I voluntarily consent to receive telemedicine services
  • I understand the benefits, risks, and limitations
